Terms and Conditions

 

Service Agreement 


  1. Five Star Driver Training and Safety Consulting, herein referred to as Five Star, conducts all Federally required ELDT training, any additional training required by State or Local regulations is the responsibility of the party contracting with Five Star.
  2. All trainees must have their Commercial Learner's Permit (CLP) prior to the start of Behind the Wheel (BTW) Training.
  3. All trainees shall complete the Theory coursework provided, and score a minimum of 80% on all assessments. Access to the Theory will become available upon payment and scheduling of BTW training.   
  4. The contracting party is responsible for providing the vehicle to be used for training, this vehicle should be of the same size, style, and type that the trainee will be conducting their Commercial Drivers License (CDL) testing in.
  5. Trainees are to be covered under the insurance of the contracting party.
  6. The contracting party is responsible for providing lot space to conduct range training in, this lot does not need to be owned by the party, but they do have to have permission to use the property.
  7. Five Star greatly encourages the party to work with the trainee prior to their ELDT training in order to instill the basics of driving skills, this will help the ELDT process go smoother.
  8. The contracting party understands that successful completion of the ELDT training does not guarantee successful completion of the CDL Exam. The party must continue to work with the trainee after the ELDT training has been completed. There will be no refunds issued for trainees who successfully complete ELDT training, but fail to complete CDL Testing.
  9. Trainees must certify that they will comply with the U.S. Department of Transportation regulations in parts 40, 382, 383, and 391, as well as State and/or local laws, related to controlled substances testing, age, medical certification, licensing, and driving record.
  10. A photocopy of the trainees CLP must be obtained by Five Star for both their records, and to record in the federal Training Provider Registry (TPR).

Cancelation Refund Policy

Refunds will be issued based on when the cancelation is made:

1) If cancelled more than 7 days prior to the scheduled ELDT training, a full refund minus the cost of the ELDT Theory will be provided

   A) ELDT Theory Cost

       i) Class B CDL: $125.00

       ii) Class B to A CDL: $125.00

       iii) Passenger Endorsement: $100.00

 2) If cancelled more than 3 days in advance, but less than 7 days, a refund of ELDT training cost will be provided, however the travel and theory cost will be forfeit.

3) If cancelled less than 3 days from the scheduled ELDT training, no refunds will be issued.
